WHAT IS EFMP PROGRAM?
The Exceptional Family Member Program (EFMP) is a mandatory U.S. Department of Defense enrollment program that works with other military and civilian agencies to provide comprehensive and coordinated community support, housing, educational, medical, and personnel services worldwide to U.S. military families with special needs.
Assignment code Q will be attached to the member who has EFMP family member.
WHO IS ELIGIBLE?
· Potentially life-threatening conditions and/or chronic medical/physical conditions.
· Current or chronic (6 months or greater) mental health conditions or treatment within the last five years.
· Requires more than annual follow up with medical providers or specialist to maintain current health conditions.
· Requires an Individual Education Plan (IEP) or Individual Family Service Plan (IFSP) through school system.
· Requires ECHO service and/or speech/occupational/physical Therapy.
WHAT IS EFMP'S MISSION?
· Proactively identify family members who may have special medical, dental or educational needs
· Ensure medical/educational service requirements for these
families can be met when sponsors are reassigned
· Ensures smooth transition of medical care during family member relocation process
· Provides healthcare coordination to family members before, during and after relocation( case management, PCM to PCM exchange, assignment coordination)
WHAT IS FAMILY MEMBER RELOCATION CLEARANCE?
The Air Force ensures personnel are assigned to locations where family member's needs are met. The Family Member Relocation Clearance screening identifies special needs and determines the availability of services at projected location. The EFMP process does not affect TDY or mobility status. Each member can initiate this process 6 months before DEROS and this process may take 30 days after turning in the paperwork to EFMP-M office.

There are three branches in the EFMP program:
EFMP-M (Medical)
EFMP-A (Assignments)
EFMP-FS (Family Support)
HOW CAN WE ASSIST YOUR FAMILY MEMBER RELOCATION
CLEARANCE PROCESS?
EFMP-M office will assist you for the next PCS move to the gaining location. When you have your assignment please fill out the AF form 4380 and stop by the Tricare office to see if any further process is required for your family member clearance.
