Ground Accident Investigation Report

On 1 July 2021, Lieutenant General Jon T. Thomas, Deputy Commander, Pacific Air Forces (PACAF), appointed Colonel Philip C. Dorsch to conduct a ground accident investigation for a fire that occurred on 19 April 2021 at Building 354, a water treatment facility the Mishap Building, Osan Air Base (OAB), Korea. The investigation was conducted at OAB, Korea, from 21 July 2021 through 20 August 2021. Additionally, Lieutenant General Thomas appointed the following board members: a Major Legal Advisor, a Technical Sergeant Fire Protection and Prevention Specialist, a Staff Sergeant Electrician, and a Staff Sergeant Recorder. He also appointed a Technical Sergeant Water, Fuels, and Systems Maintenance (WFSM) subject matter expert. 

In accordance with Air Force Instruction 51-307, Aerospace and Ground Accident Investigations, this ground accident investigation board (GAIB) conducted a legal investigation to inquire into all the facts and circumstances surrounding this Air Force ground accident, prepare a publicly-releasable report, and obtain and preserve all available evidence for use in litigation, claims, disciplinary action, and adverse administrative action.
